We are seeking a per diem Nurse Practitioner or Physician Assistant for coverage with Skilled Nursing Facilities (SNF) in the Canaan, CT area. Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• Training as an APRN (Family, Adult/Geri Nurse Practitioner) or PA
• At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)

Responsibilities

• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
• Develop plans for further evaluation of health needs in consultation with the physician(s) and care team members.
• Provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols.
• Identify problems outside of their scope of practice and refer appropriately to physicians.
