About The Position

This position involves providing direct and indirect nursing services to patients and families in a hospice setting. The role requires the nurse to float between four Hospice Home locations and may involve fieldwork. The working hours are from 7 PM to 7:30 AM, with a minimum of 4 shifts in a 4-week schedule, including weekends and holidays as needed.

Requirements

• Graduate of nursing school.
• 3 years of clinical experience.
• California Registered Nurse (RN) license.
• AHA Basic Life Support for Healthcare Professional certification.
• Valid California driver's license.

Responsibilities

• Provide direct and indirect nursing services to patients and families.
• Delegate and assign tasks to team members, ensuring clear communication of expectations.
• Assess appropriateness of physician orders and question when necessary.
• Document patient care according to department guidelines and policies.
• Establish therapeutic relationships with patients and families, anticipating and addressing potential barriers.
• Act as a patient advocate, supporting their needs and facilitating communication with healthcare providers.
• Coordinate patient education and discharge planning with the healthcare team.
• Utilize the nursing process to provide individualized, goal-directed care.
• Continuously evaluate interventions and adjust care plans as needed.
• Serve as a preceptor and teacher for new nurses and students.
