You will manage complex cases and provide high-level care that keeps our clients safe and healthy at home.

Requirements

  • Nursing Background: 5+ years of RN experience in Rehab, Long-Term Care, or Assisted Living.
  • Case Management: 5+ years of experience in client care planning and management.
  • Credentials: Active Rhode Island RN License, CPR Certification, valid driver's license, and an insured vehicle.
  • Critical Thinking: The ability to rapidly assess symptoms and make high-stakes decisions in a home environment.
  • Ability to lift 50 lbs
  • assist with transfers
  • travel across Providence, Kent, and Washington Counties

Responsibilities

  • Clinical Oversight: Initiate and manage new cases, conduct home evaluations, and provide clinical supervision for our team of CNAs.
  • Complex Nursing Care: Administer physician-prescribed medications, perform advanced wound care, and manage medical devices.
  • Patient Advocacy: Accompany and advocate for clients at physician appointments and lead hospital discharge planning for seamless transitions.
  • Education & Empowerment: Provide essential teaching to patients and families to support informed decision-making and self-care.
  • Documentation: Utilize EMR software to communicate clinical updates to the DON, ADON, and families in real-time.
