Northeast Arc is seeking a compassionate and detail-oriented Per Diem Nurse Evaluator to support individuals enrolled in our Personal Care Management (PCM) Program throughout Westford, Dracut, Burlington, and Lowell, MA . About Northeast Arc At Northeast Arc , we partner with individuals with disabilities and their families to help them live as full members of their communities. Through our Personal Care Management (PCM) Program , we support approximately 5,000 individuals across Northeastern Massachusetts, Greater Boston, and MetroWest by helping them access and manage personal care services that promote independence and quality of life. The PCM program, a MassHealth initiative , allows individuals with long-term disabilities to remain safely in their homes by hiring and directing their own Personal Care Attendants (PCAs). Our interdisciplinary teamincluding Skills Trainers, Nurses, and Occupational Therapistsworks collaboratively to assess needs, develop care plans, and empower consumers to successfully manage their services. Position Summary The Nurse Evaluator plays a vital role in conducting clinical re-evaluations to determine ongoing eligibility for PCA services and the appropriate level of care. Evaluations are completed in collaboration with Occupational Therapists and reflect Northeast Arcs commitment to person-centered, community-based care.

Requirements

  • Active, unrestricted RN or LPN license in the Commonwealth of Massachusetts
  • Minimum of 2 years of nursing experience
  • Experience providing direct care to elderly individuals or adults with disabilities
  • Completion of the MassHealth PCM Training Program (or willingness to complete)
  • Current CPR certification
  • Strong understanding of community supports and services
  • Valid drivers license and reliable transportation

Nice To Haves

  • Home care or community-based experience strongly preferred

Responsibilities

  • Conduct in-person clinical assessments in the consumers home or place of residence alongside an Occupational Therapist
  • Evaluate physical, cognitive, and functional abilities to determine eligibility and appropriateness for PCA services
  • Assess needs related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Ensure evaluations accurately reflect the consumers level of required physical assistance
  • Complete all documentation in a timely, thorough, and compliant manner
  • Collaborate closely with Occupational Therapists, Skills Trainers, and the broader PCM team
