CommuniCare+OLE is a network of federally-qualified health centers established in 2023, resulting from the union of OLE Health and CommuniCare Health Centers. The organization provides comprehensive care, including medical, dental, behavioral health, and substance use treatment, to over 70,000 individuals across Napa, Solano, and Yolo Counties. The Physician Assistant or Family Nurse Practitioner will be responsible for delivering quality primary health care to patients, collaborating with healthcare staff in a team approach. This position is per diem and located in Fairfield or Napa, CA, with an hourly rate of $75.81, depending on experience.
