About The Position
The Per Diem Mobile Integrated Health Nurse plays a crucial role in the Vashon community by being part of a dedicated treatment and outreach team. This position is designed for a registered nurse who is passionate about providing health services directly to patients in their homes. The primary responsibility of the Per Diem Mobile Integrated Health Nurse is to respond to scheduled patients, ensuring they receive the necessary care in a timely manner. This involves not only delivering nursing care but also assessing the patients' needs and connecting them to acute care services when required. In addition to responding to scheduled visits, the nurse will actively work to identify and engage high-needs individuals within the community. This includes understanding the social determinants of health that may affect these individuals and connecting them to appropriate medical, behavioral, and social services. The role requires a compassionate approach, as the nurse will be working with vulnerable populations who may face barriers to accessing healthcare. The Per Diem Mobile Integrated Health Nurse will also collaborate with other healthcare professionals and community organizations to ensure a holistic approach to patient care.
