Requirements
- Graduate of an accredited LVN Program
- Current LVN License
- License Vocational Nurse in the State of Texas
- Current BCLS
- Ability to travel throughout the greater Houston area to facilities and patient homes
- Driver’s license and access to reliable transportation and current insurance coverage
Nice To Haves
- 1+ years of Home Health experience
- Care Management/ Disease Management
- EPIC experience
- Windows based computer skills
Responsibilities
- The LVN provides direct nursing care to an assigned group of patients, under the appropriate supervision of a Provider, Registered Nurse, by applying clinical nursing knowledge, skills, and ensuring the safety and comfort of patients and families according to legal, organizational and professional standards
- The LVN prioritizes and makes judgments concerning basic needs of multiple patients to organize care
- Assists with outreach initiatives and goals set forth by the Primary Care at Home leadership
- Ongoing liaison between patient, multispecialty care departments and approved Facilities
