About The Position

The Per Diem Hospice Field LVN at Calstro Hospice Inc. is responsible for providing care to terminally ill patients in their homes under the supervision of a Registered Nurse (RN). This role involves implementing individualized care plans, educating patients and families about the end-of-life process, and ensuring safety and proper medication administration. The position requires strong organizational and communication skills to effectively manage patient care and collaborate with administrative and field staff.

Requirements

• Currently licensed as an LVN in the State.
• Three years of nursing experience.
• Current BLS provider card.
• Great verbal and written communication skills.
• Strong organizational skills.
• Basic computer competence.
• Must be a licensed driver with an insured automobile.

Nice To Haves

• Bilingual in English and Spanish preferred.

Responsibilities

• Care for terminally ill patients in their homes under RN supervision.
• Implement and educate patients and families on end-of-life processes.
• Administer medications and treatments as per MD orders.
• Document patient care accurately and follow company policies.
• Participate in PRN visits as needed.
