Good physical stamina and mental health. Basic Life Support Certification (BLS) required. Excellent communication skills and ability to follow written instructions. Self-directed and able to work with little direct supervision. Demonstrates tact, patience, and good personal hygiene. Valid driver's license and insured automobile. We Offer 6 sick days, 401(k) with company match This position is a Pay per visit compensation model. Pay per visit ranges from $50-$80/visit, commensurate with experience and depending on type of visit. As an organization, we set the highest quality standards for home health services. As a Per Diem Home Health Medical Social Worker, you will play a vital role in providing care to our patients. You will assess and evaluate patients' needs for community resources and in-home support services, assist in implementing the plan of care, support patients with advance directives, and maintain accurate clinical notes. Our electronic medical records system allows you to complete all charting inside the patient's home efficiently. Requirements
