About The Position

AdvisaCare is a fast-paced Home Health and Hospice Agency looking for compassionate, dynamic, energetic, organized, and efficient professionals to join our team as Home Health and Hospice LPNs. This role is perfect for licensed practical nurses who value home health and hospice care. You will provide skilled nursing care to patients in their homes, offer emotional support to patients and their families, and conduct supervisory visits to observe the performance of assigned team members. You will also participate in interdisciplinary group meetings to develop and revise patient/family care plans.

Requirements

• Current Michigan License in good standing.
• Minimum 1 year of home health and hospice experience.
• Empathetic approach to patients and caregivers.
• Ability to participate in field visits daily and attend weekly office meetings as indicated.
• Ability to adapt to patient’s status and needs.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial, and environmental needs.
• Implement the individualized plan of care and make revisions as needed.
• Ensure compliance with AdvisaCare Policies and Procedures and Medicare compliance.
• Consult with and educate patients/families on disease processes, self-care techniques, and end-of-life care.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical and progress notes and participate in IDG conferences.
• Participate in Home Health and Hospice performance improvement programs.
• Maintain a high level of customer satisfaction with clients, families, and caregivers.
• Communicate information using current processes and technology within the organization.
