Per Diem Home Care LPN
About The Position
Eminence Home Care is seeking a compassionate and skilled Per Diem Home Care LPN to join our dedicated team of healthcare professionals. In this role, you will provide essential nursing care to clients in the comfort of their homes, ensuring they receive the support they need to maintain their health and independence. As a Per Diem LPN, you will have the flexibility to choose your hours, making it an ideal position for those looking to balance work with other commitments. In this position, you will assess patients' medical needs, administer medications, provide wound care, and assist with daily living activities. Your ability to establish rapport with clients and their families will be crucial in delivering compassionate, patient-centered care. You will collaborate closely with RNs, caregivers, and the broader healthcare team to develop and implement individualized care plans.
Requirements
- Active LPN license in the state of Connecticut.
- Previous experience in home care or similar setting preferred.
- Strong clinical assessment and nursing skills.
- Excellent communication and interpersonal skills to engage with clients and families.
- Ability to work independently and manage time effectively.
- Reliable transportation and valid driver's license.
- Must have current CPR certification.
- Flexible schedule to accommodate varied per diem shifts.

Responsibilities
- Administer medications and provide treatments as prescribed.
- Conduct nursing assessments to monitor patient health status.
- Assist patients with personal care activities, including bathing, grooming, and mobility.
- Educate clients and families on medication management and health maintenance.
- Document care provided and maintain accurate patient records.
- Communicate effectively with the healthcare team and act as a liaison between patients, families, and providers.
- Maintain a safe and supportive environment for clients.
