Job Description: Per Diem Heart Failure Advanced Practitioner –Heart Failure Clinic (outpatient) BILH Lahey Hospital and Medical Center Department of Cardiovascular Medicine is seeking a skilled and highly motivated NP or PA to work in a per diem role within our busy, well-established, Heart Failure Clinic. As an Advanced Practitioner in the Heart Failure Clinic, you will play a key role in the management and care of patients with complex heart failure. You will be responsible for assessing heart failure patients within the clinic setting and developing a comprehensive treatment plan to ensure optimal outcomes. This role requires a compassionate and hardworking individual with a strong foundation in cardiology and, specifically, exceptional knowledge of advanced heart failure care.

Requirements

  • Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • At least 2 or more years of experience in outpatient cardiology setting required.
  • Strong clinical knowledge and the ability to manage complex cardiac conditions.
  • Excellent communication skills and the ability to work well within a multidisciplinary team.
  • Compassionate, patient-focused attitude with a commitment to providing high-quality care.

Nice To Haves

  • Previous experience in heart failure management strongly preferred.

Responsibilities

  • Perform comprehensive patient assessments and diagnostic evaluations related to heart failure.
  • Develop and implement individualized care plans, including pharmacologic and non-pharmacologic interventions.
  • Monitor patient progress, adjust treatment plans as needed, and provide ongoing education to patients and families.
  • Collaborate with cardiologists, nurses, and other healthcare professionals to provide coordinated care.
  • Order and interpret diagnostic tests, including echocardiograms, EKGs, and lab work.
  • Provide patient education regarding heart failure management, lifestyle modifications, and medication adherence.
  • Document all patient interactions and maintain accurate and up-to-date medical records.
