About The Position

The essential functions of the job for the Care Manager require compliance with standards of business conduct in accordance with federal, state, and local health and regulatory standards and guidelines. The position necessitates a high school diploma or GED, the ability to work well with peers, and the utmost compassion and dignity towards residents, adhering to the Resident Bill of Rights. In exigent circumstances, the employee may be subject to call back during emergencies. Adherence to attendance policies and the Employee Handbook is mandatory. The Care Manager must be proficient in reading, writing, speaking, and understanding English, and maintain high standards in caring for residents and their families. Compliance with all facility policies and procedures is required.

Requirements

  • Must possess a high school diploma or GED.
  • Must provide annual verification of a negative TB skin test.
  • Must be able to move intermittently throughout the workday.
  • Must cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that enable these senses to function adequately.
  • Must meet general health requirements set forth by facility policies.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and agitated residents.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet.
  • Must be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

  • Supervise and care for residents on a daily basis according to work list and assignment.
  • Provide assistance with activities of daily living according to resident’s individualized Care Plan.
  • Observe and record assistance with prescribed medications.
  • Report refusals and/or reactions to care or medications to supervisor on duty.
  • Direct residents and family members to appropriate staff for other needs or information.
  • Assist with ordering room service for residents when needed.
  • Report and record observations of change in resident condition to director/supervisor.
  • Provide assistance in the dining room as needed.
  • Answer emergency calls promptly and courteously, completing required paperwork afterward.
  • Retrieve soiled linen and personal clothing from resident living areas.
  • Responsible for general upkeep and cleanliness of a resident’s apartment/room.
  • Report any unsecured throw rugs, extension cords, and other fire/fall hazards.
  • Document clearly and objectively on residents.
  • Participate in in-service educational programs as required.
  • Record all care given and refusals.
  • Maintain required safety/building checks.
  • Obtain resident weights as needed.
  • Maintain universal precautions; bloodborne pathogens and biohazard precautions.
  • Promote positive public relations for the Community.
  • Provide assistance with toileting, personal care, and changing incontinence products.
  • Assist with the use of durable medical equipment (DME).
  • Use good body mechanics to provide occasional assistance with transfers.
  • Assist residents to perform passive range of motion (ROM) exercises.
  • Assist residents with braces, splints, slings, hearing aids, artificial breasts, dentures, and TEDS stockings.
  • Perform other duties as required.
