About The Position

Provide direct service and routine nursing aide care in a clinically intensive program designed to meet the needs of clients who have both medical needs and psychiatric needs and/or developmental disabilities. Provide assistance with activities of daily living and medication administration services under the Medication Administration Program (MAP) guidelines. Use rehabilitation values, principles and techniques, including skills training to help program clients overcome barriers to living independently. This position may be eligible for a differential pay rate depending on assigned work hours, location, area, or division.

Requirements

  • One to two years of job-specific experience in human services field preferred.
  • A high school diploma or GED is required.
  • May be called on to drive for Vinfen using either a Vinfen van or personal vehicle. A valid driver’s license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver’s license for at least six months, and must be able to pass a driver’s screening background check, which is subject to annual checks. If driving, must maintain a safe driving record. If using a personal vehicle, must possess and maintain adequate insurance.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position. Must be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift. Ability to operate a computer and other office equipment such as a calculator, copier, and printer. Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways. Ability to assist clients with tasks of daily living. Ability to remain in a stationary position 50% of the time as needed. Ability to bend, reach, file, sit, stand, and move around the facility. Ability to speak, hear, and communicate with clients, staff, and external representatives. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
  • First Aid is required within two weeks of hire
  • CPR is required within two weeks of hire
  • MAP is required within 150 days of hire
  • Safety Care is required within 90 days of hire
  • CNA in state of service required

Responsibilities

  • Provide assistance with activities of daily living according to each client’s/persons served treatment plan that includes, but is not limited to community membership, community relationships, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, passive range of motion exercises as needed or ordered by primary care provider.
  • Responsible for a variety of daily tasks with clients/persons served. This includes rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress and other related tasks.
  • Provide role model and support to clients/persons served. Assist individuals in becoming fully integrated into their communities.
  • Take and document client’s/person’s served temperature, pulse, respirations, blood pressure, skin care, toileting, and/or passive range of motion exercises as ordered by primary care clinician.
  • Implement and document individual treatment support plans.
  • Store, administer, document, and dispose of client’s/persons served medication consistent with Medication Administration Program (MAP) requirements, funding source regulations and company policy, if applicable. Ensure program compliance with Vinfen MAP protocols.
  • Transport, assist, and accompany clients/persons served to medical, dental, and other appointments as directed by shift/program manager/administrator.
  • Create an environment in both the client’s/person’s served room and common area that is safe, clean, welcoming, and respectful to individuals, families, and neighbors. Ensure that the environment meets all safety and regulatory standards.
  • Act as an advocate for clients/persons served.
  • Assess and report any changes in client’s/persons served medical condition or mental status to supervisor.
  • Participate in trainings, staff meetings, and other meetings as requested.
  • Work effectively and safely with assistive equipment that includes, but is not limited to wheelchairs, Hoyer lifts, and other medical equipment. Maintain client’s/persons served medical equipment and supplies that includes, but is not limited to wheelchairs, walkers, and Hoyer lifts. Work effectively and safely with medical equipment that includes but is not limited to glucometers, nebulizers, and oxygen.
  • Document services provided to clients/persons served as required by Vinfen Policy.
  • Complete all service delivery, MAP, and administrative documentation in a thorough and timely manner.
  • Contribute to program’s quality improvement goals and objectives.
  • Use crisis intervention techniques when necessary. Access on-call back up when needed. Complete incident documentation as needed.
  • Ensure maintenance and safety of physical site. Oversee and participate in the daily living and cleaning of the program, inside and outside with clients/persons served. This includes cooking, meal planning, grocery shopping, general housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Adhere to all company and divisional and program specific policies and procedures, including safe work practices, worker’s compensation, universal precautions, and exposure control. Comply with accreditation and licensing standards, as well as regulations.
  • Perform other related duties, as required.
