Requirements

  • A High School Diploma/GED and at least 1.5 years of relevant experience in a behavioral health setting; OR An associate’s degree in a behavioral health field; OR A bachelor’s degree in the behavioral health field; OR A non-behavioral health field bachelor’s degree and one year of relevant experience in a behavioral health setting.
  • Must pass DCS Central Registry check without any disqualifying events.
  • Must be at least 21 years of age.
  • Background check.
  • Minimum typing speed of 25 wpm.
  • Valid Arizona Level One Fingerprint Clearance Card or ability to obtain an Arizona Level One Fingerprint Clearance Card.
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR).

Responsibilities

  • Conducting intakes for new patients, developing individualized ISPs and providing orientation to the program.
  • Physical walkthrough and explanation of the expectations and requirements for the program for new patients.
  • Conducting contact at least 2x weekly to assess ongoing needs and make adjustments to the treatment plan progress and goals.
  • Connect the client with appropriate community resources and supports, including outpatient medical, child-care, employment, education, housing, and behavioral health supports and services.
  • Educating and supporting the client through reintegration into the community, being successful in employment, and obtaining permanent housing.
