About The Position

The Wellness & Recovery Specialist is responsible for care coordination activities focused on the Wellness and Recovery of members. Location: Virtual: This role enables associate to work virtually full-time, with the exception of required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Please note that per our policy on hybrid/virtual work, candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law. Candidates must reside in Tennessee

Requirements

  • Requires AA/AS and minimum of 2 years of experience in health services or behavioral health field or in a peer support services role; or any combination of education and experience, which would provide an equivalent background.
  • Peer Specialist Certification required.
  • Candidates must reside in Tennessee

Nice To Haves

  • One year experience working as a Peer Support Specialist, after receiving certification, or related equivalent experience.
  • Experience working with computers toggling back and forth through multiple screens.
  • Experience with researching resources.
  • Telephonic customer service.
  • Knowledge of the Recovery process and the ability to facilitate recovery using established standardized mental health processes.
  • Knowledge and skill to teach and engage in basic problem-solving strategies to support individual clients in self-directed recovery.
  • Knowledge of the signs and symptoms of mental illness & substance use disorders.
  • Knowledge and skills sufficient to use community resources necessary for independent living and ability to teach those skills to other individuals with severe mental illness.
  • Knowledge of how to establish and sustain self-help (mutual support) and educational groups by soliciting input from the mental health consumers on their strengths and interests.
  • Proven experience in health coaching and motivational interviewing techniques.
  • Demonstrated ability to work proficiently across multiple computer platforms simultaneously.
  • Excellent typing skills with attention to detail.
  • Strong communication skills, both written and verbal.
  • Knowledge of care-coordination and case management concepts strongly preferred.
  • BA/BS or MBA preferred.

Responsibilities

  • Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.
  • Develops and implements provision of psychiatric discharge planning education at Recovery and Resiliency sites.
  • Collaborates with Stabilization Teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.
  • Acts as a resource for staff on decision making and problem solving.
  • Initiates and maintains contact with assigned individuals and providers to determine member's response to services.
  • Teach and role model the value of the member’s recovery experience.
  • Models effective coping techniques and self-help strategies.
  • Serve as a recovery agent by providing and advocating for recovery-based services to assist the members in daily living.
  • Identify opportunities for engagement of members in forming a supportive, peer recovery network.
  • Assist members in working with their case manager or treatment team in determining the steps he/she needs to take to achieve these goals and self-directed recovery.
  • Assist members in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups.
  • Initiates and maintains contact with assigned members to determine members’ engagement and response to services.
  • Assist members understand Peer Recovery resources available within the local community.
  • Assist members in identifying recovery resources, supports, remove barriers, and facilitate connections in the local community.
  • Collaborates with interdisciplinary teams as a member advocate in discharge planning education, resolution of barriers, and service transitions.
  • Acts as a resource for staff on decision making and problem-solving as it relates to the peer role.
  • Utilize tools such as the Wellness Recovery Action Plan (WRAP) to assist clients in creating their wellness and recovery plans.
  • Assist clients in developing empowerment skills and combating stigma through self-advocacy.
