This position provides professional case management work for consumers suffering from serious mental illness or emotional disturbance in a community setting. A caseload is assigned. The employee is responsible for arranging/providing services to consumers that encourage and promote healthy functioning, recovery efforts, consumer independence/self-care and responsibility. The employee provides assistance to ease an individual’s immediate and continued adjustment to community living or within a family or school system, by coordinating the delivery of mental health services with services provided by other agencies. The peer support worker serves on an interdisciplinary team and ensures that treatment goals are developed and implemented for each consumer, and clinically appropriate services are delivered in accordance with policies and contractual requirements. He/she monitors consumer progress in treatment programs. Work is under the general review of an administrative superior, leaving considerable latitude for independent judgment and discretion within an established framework of policies and state regulations.
