At FBH we have a variety of client services, including outpatient, inpatient, community outreach, crisis services, partnerships with schools, law enforcement and more. Our evidence-based, culturally appropriate, trauma informed behavioral health services address the diverse needs of those we serve. The onsite Permanent Supportive Housing Team serves Trueblood class members-individuals with behavioral health conditions involved in the criminal justice system-residing in permanent supportive housing. This role is essential to maintaining a safe, stable, and trauma-informed environment that promotes recovery, dignity, and long-term housing success. Maintain a safe and welcoming environment for resident through regular monitoring an de-escalation support. The Permanent Supportive Housing Team will: Build a trusting relationship with residents and provide emotional and practice support. Assist residents with daily living activities, including hygiene, meals, and medication reminders. Support residents in accessing services such as behavioral health, legal aid, and employment programs. Document incidents, resident interactions, and service coordination in compliance with program standards. Collaborate with case managers, peer support specialists, and health care providers. Uphold trauma-informed care, harm reduction, and housing first principles. Peer Support Specialists promote community integration, resiliency and quality of life objectives with clients and/or family members. When appropriate and helpful to the client, Peer Support Specialists share relevant personal experiences that foster hope, resiliency and decrease stigmatization of mental illness. Peer Support Services are provided to individuals and families of all ages. These services may be provided on an individual or group basis.
