About The Position

The Peer Support Specialist role is focused on in-reach, engagement, education, and support for adults with serious mental illness (SMI) or serious and persistent mental illness (SPMI). This position aims to inform individuals about community-based mental health services and supported housing options, including tenancy support services and rental assistance. The Peer Support Specialist will conduct ongoing in-reach to educate members about community options, including transitioning to supported housing, and the benefits and services available to them. The role also involves informing members about Medicaid, Special Assistance, and other services they may be eligible for. Additionally, the Peer Support Specialist will facilitate opportunities for members to connect with others who have disabilities and are receiving services in inclusive settings.

Requirements

  • High school diploma or GED.
  • 1-2 years of experience working with the mental health/substance use population.
  • NC Certified Peer Support Specialist certification.

Responsibilities

  • Conduct minimum in-reach visits per day to meet weekly requirements.
  • Maintain a caseload ensuring every person has a Peer Support Specialist visit prior to reaching 90 days.
  • Coordinate face-to-face meetings with members and guardians to discuss interest in supported housing.
  • Educate members about the benefits and financial aspects of community-based inclusive settings.
  • Document all Peer Support Specialist activities and track outcomes of in-reach efforts.
  • Participate in necessary activities at the NC state level, including training and information sessions.
  • Work closely with other Vaya staff to ensure smooth transitions and address challenges.
