About The Position

The MAT (Medication Assisted Treatment) Peer Support Advocate works under the direction of the Adult Services Supervisor to provide consistent social support, case management and advocacy services for clients accessing MAT at CommuniCare+OLE and other collaborating agencies. Support for clients includes but is not limited to: strengths-based efforts to facilitating communication between departments/agencies, accessing information, connecting with community, goal-setting, and management of daily tasks. Requirements

  • High school diploma or equivalent required
  • Lived experience as someone who has successfully navigated one / all of the following: mental health system, substance use disorder treatment, homelessness, or incarceration and achieved 2 years of stability.
  • Must provide current driver’s license and auto insurance, ability to travel in Yolo County (West Sacramento, Davis, Woodland).
  • Able to engage and provide services to patients by way of HIPAA-compliant telehealth/video-based services and fully participate in video-based meetings and trainings.
  • Must have ability to effectively use telephonic translation services when client language preference dictates.
  • BLS Certification required.

Nice To Haves

  • Previous personal experience completing a diversion program strongly preferred.
  • Experience supporting those who have involvement in the justice system and may have a range of mental health conditions, including those who have severe and persistent conditions and/or experiencing substance use disorders.
  • Bilingual English/Spanish strongly preferred.

Responsibilities

  • Provides in-person services and social support, modeling, and advocacy for clients experiencing a range of mental health symptoms and/or substance use disorders.
  • Collaborates with physical and mental health providers, including Yolo County Health and Human Services Agency.
  • Demonstrates comfort with sharing and utilizing aspects of their own personal story to instill hope, develop relationship, promote program engagement, and offer mentorship. Maintains professional boundaries at all times.
  • Support clients with reducing / eliminating barriers to stability and wellness, helps in the coordination services and actively supporting linkage.
  • Ability to develop relationships with clients, respect client choice, and build and maintain trust with a diversity of clients from different backgrounds and life experiences.
  • Manages a fast-paced workday and is able to meet direct service requirements for program.
  • Completes documentation of all client interactions and services within 48-hours of services in accordance with working hours, all documentation must comply with relevant guidelines. Participate in peer chart reviews and other quality improvement efforts.
  • Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and patient service standards.
  • Participates in a team-based approach to care including staff meetings, case review, consultation/supervision sessions and other forums as required. Engages in constructive problem solving and conflict resolution.
  • Creates a supportive discharge or program completion process, in collaboration with other members of the team
  • Responsible for remaining within established ethical standards, including those of confidentiality and mandated reporting.
  • As safe and appropriate to do so, transport clients or support with transportation arrangements.
  • Attend trainings/conferences as arranged.
  • Performs other duties as assigned.
