About The Position

The Bridger/Peer Specialist is responsible for providing self-help and empowerment tools to participants in the community through participant led groups as well as providing individual services.

Requirements

  • Good verbal and written communication skills
  • Prior Peer experience or Peer education training
  • High School Diploma or GED (General Equivalency Diploma) required.
  • Internet Savvy with knowledge of Microsoft Word and Excel
  • Experience with entitlements (SNAP Benefits, SSDI/SSI, Medicare/Medicaid, HRA)
  • Must be APS (Academy of Peer Services) certified or Provisional submission of application for certification within 6 months of hire. You are required to maintain your certification through continuing education courses and events.
  • An approved background check is required prior to employment.
  • Baltic Street Wellness Solutions will require selected candidates to submit proof of complete vaccination against COVID-19 and having received a booster shot. If a candidate has a qualifying medical condition or religious exemption that contraindicates a COVID-19 vaccination, an exemption form should be requested from Human Resources.

Responsibilities

  • Model and promote the principles of recovery/self-help.
  • Manage a caseload within program standards.
  • Input information for 2010E application into PACT system and referral sources
  • Ability to navigate program systems. (AWARDS)
  • Develop a person-centered Individual Recovery Plan with the participants and their chosen support input.
  • Develop community asset-based mapping with the participants and their chosen support team.
  • Assist clients with entitlement and provide linkage, and/or referrals to such services (SNAP), Medicare, Medicaid, SSI, SSD, reduced fare MetroCard and HRA etc.
  • Empower participants to advocate for themselves and promote efficiency.
  • Advocate for other needed services and assist participants to negotiate service systems.
  • Develop a strong relationship with clients’ treatment team.
  • Conduct wellness activities.
  • Conduct and or co-facilitate weekly or scheduled housing groups with accurate information.
  • Able to problem solve office and client issues independently/collectively.
  • Conduct Chart reviews as directed by the program manager.
  • Submit all programs and participant documentation in a timely manner.
  • Knowledgeable in SAMHSA 8 dimensions of wellness.
  • Able to enact WRAP (Wellness Recovery Action Plan) and/or psychiatric directives in time of crisis.
  • Demonstrate aptitude in a cultural humility diverse environment.
  • Exhibit encouraging dialogue when communicating with clients.
  • Maintain all charts within audit standards.
  • Attend staff meetings and present cases, as necessary.
  • Actively participate in ongoing training as needed to meet all certification standards and credentialing policies.
  • Required to work in the field if necessary.
  • Comply with all program regulations.
  • Experience working in a Crisis Management Model.
  • Ability to collaborate and coordinate with multidisciplinary teams, including Behavioral health professionals, social workers, and community organizations.
  • Must be available for an On-Call schedule.
  • Must be able to work a flexible schedule with evenings and weekends included.
  • Support the participants in community reintegration.
  • Continue to support participants for up to six months to ensure success of reintegration.
