About The Position

Provide education and support to clients in recovery for a mental health disorder. Participate in outreach to community partners to help coordinate services for clients in order to support their recovery. Maintain regular contact with clients to monitor and assess progress.

Requirements

  • High School Diploma or equivalent.
  • Lived experience with Mental Health Disorders.
  • Experience working with clients with history of mental illness.
  • Peer Support Specialist certification or ability to obtain within first three months of employment.
  • Valid driver’s license and current auto insurance may be required.

Responsibilities

  • Assist staff with caseloads through regular client contact in community and program setting.
  • Track and assess progress as determined by program guidelines.
  • Complete reports as requested by our partners.
  • Assist with the reduction of recidivism when working with hospitals, halfway housing and family care homes.
  • Help to develop and implement Individual Treatment Plans.
  • Efficiently operate personal computer to enter, edit and/or retrieve data using agency electronic health record.
  • Assist clients in accessing community resources, including, but not limited to food, clothing, supportive housing, Medicaid, job readiness and/or placement, public assistance, etc.
  • Facilitate site-based daily support/peer groups.
  • Visit, support, and transport clients as needed upon discharge from program.
  • Collaborate with family members, internal and external case managers, and other providers while supporting client’s stability and recovery.
  • Prepare documentation in compliance with contract, regulatory and Agency requirements.
  • Other duties as assigned.
