The Peer Recovery Support Specialist will conduct initial screenings, introduce clients to substance use disorder (SUD) and behavioral health treatment programs, and serve as a primary coach. Will help clients access additional community services

Responsibilities

  • Conduct initial brief screenings, introduce patients to SUD programs & behavioral health treatment programs, serve as primary coach for clients, and maintain ongoing contact. Assist with access to other community services.
  • Demonstrate empathy, caring, or concern to bolster patient’s self-worth and confidence
  • Facilitate contacts with other friends and allies in the community through meetings, sober, or other events to promote learning of social and recreational skills, create community, and acquire a sense of belonging, helping our patients connect with as many healthy and meaningful affiliates as we can.
  • Collaborate with SUD team, providers, and Blue Stone Health staff to provide comprehensive, patient-centered care
  • Document interactions with patients
  • Remain in full compliance with 42 CFR Part 2, which guarantees confidentiality for people seeking treatment for substance use disorders from federally assisted programs, as instructed within the PRSS state certification classes
  • Attend required trainings and meetings provided by the SOR HSS Learning Collaborative
  • Obtain additional education in mental health first aid
  • Connect with Peer Recovery Support networks for support, mentorship and as an outlet to share and process experiences.
  • Visit patients that drop out of the SUD program/miss return follow up visits
  • Go to the hospital to visit hospitalized patients/jail to visit incarcerated - get them set up to get back in care when released.
  • Have a working knowledge of where substances are frequently obtained on the street aka “hotspots”
  • Visit hotspots to recruit patients and distribute Narcan, test strips for xylazine and fentanyl, & flyers for treatment
  • Provide Narcan medication and trainings for patients and their specified friends and family members
  • Connect patients with resources for spiritual community and study, and classes on health, nutrition, cooking, exercise, etc.
  • Share knowledge and information and/or provide life or vocational skills training
  • Offer direct patient support, such as food and beverage, with a limited budget of $10/day per patient, while providing opportunity to connect with patient and develop rapport
  • Calling the pharmacy or speaking with them in person if there are difficulties picking up the prescription
  • Assist patient to arrange rides through insurance or providing rides to complete treatment or life skills, to rehab, inpatient treatment
  • Assist patients with providing lab results for telehealth visits: Deliver oral fluid testing kits, provide a ride to the lab, collect urine specimens from the home if the patient is unable to come in.
  • Outreach with treatment partners and facilitate transitions from inpatient to outpatient treatment.
  • Facilitate referrals to hub, by providing or arranging transportation, accompanying the patient, and assisting with intake paperwork
  • Assist Patient to Access to Community Resources: Driver’s License - Rides to DMV, assistance with forms, assist patient know where to find study guide pre-testing. Assisting to enroll in Driver’s education classes if needed.
