About The Position

Peer Partner will respond to referrals from the Warmline, Legal Services Partners, Emergency Response Social Workers or community in general, for families residing in SPA 6 who have had a child abuse and neglect report filed or are experiencing a current investigation with the Department of Children and Family Services (DCFS), Peer Partner is responsible for contacting the family within 24 hours and offering support, conducting assessments, developing a plan, linking to resources and providing parent rights education. Peer Partner will also help identify participant strengths, needs, barriers and risks and link families directly to community resources as well as legal services, as indicated. Peer Partner shall also provide emotional support to the family.

Requirements

  • Basic advocacy and engagement skills.
  • Present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Efficiently use the personal computer to include a variety or word processing software.
  • Utilize analysis, lived experience, and judgment to make decisions.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Provides services within a family-centered, culturally sensitive, ethnic informed and trauma focused model of practice and is respectful to the community and participants.
  • Lived experience with a closed case within the child welfare system (DCFS) is required
  • Bilingual (English/Spanish) is required.
  • High School diploma or equivalent
  • A valid California driver's license is required for this position, as driving is an essential function of the role and is required for field work, client crisis response and emergencies. A valid car insurance and registration is required.
  • COVID-19 vaccination required. May request medical or religious exemption if applicable.

Responsibilities

  • Responds to Warmline calls and/or DCFS and Community/Self referrals within 24 hours of receipt.
  • Effectively conduct outreach and engagement with participants and enroll in program services.
  • Conduct initial intake and assessment; develop in collaboration with family, a family service plan that identifies needed services and supports, including but not limited to behavioral health treatment, home visitation, and housing.
  • Contact community legal partners as needed for support and direction.
  • Assist participants with linkage to identified resources, including legal services, as needed.
  • Provide on-going contact with families based on program protocol and individual case plan for a minimum of six months to ensure progress and provide case management services as needed.
  • Collect required data and complete all needed documentation within the timelines provided and in an organized manner, including clear and well written progress notes.
  • Participate in case conferences and other related meetings with partner programs and agencies.
  • Maintain a working knowledge of existing community programs and resources, the services provided and eligibility requirements.
  • Maintain positive working relationships with local linkage and referral agencies.
  • Maintain caseload documentation in compliance with all agency policies and procedures as well as federal, state, county and local regulations.
  • Attends and participates in staff meetings, supervision, and all training as required.
  • Respond to requests for information in a timely manner.
  • Assist with the coordination of program events and activities
  • May be required to respond to participants during non-work hours.
