About The Position

The Peer Health Coach provides wellness training and support to families in the Primary Care Behavioral Health Integration (PCBHI) Program and facilitates skill-teaching groups. This position also provides information on wellness and will promote opportunities to enhance the ability of families served to improve their health and wellness.

Requirements

• Must be a Peer which is a person who has personal lived experience and has used mental health services (past or present)
• High school diploma or GED
• Ability to teach disease management and health maintenance curriculum
• Ability to utilize supervision to clarify clinical concerns
• Knowledge and skill in computer procedures relevant to this position
• A valid driving license with a clean driving record is required
• A high degree of empathy toward individuals with behavioral health disorders and the ability to connect with individuals of diverse ethnic and cultural backgrounds

Nice To Haves

• Certified Recovery Peer Specialist (Preferred)

Responsibilities

• Coordinate and complete health assessments within tight deadlines
• Complete client notes/records as required
• Conduct support groups focused on health maintenance and prevention on medical disorders
• Provide education on the recovery process, including conducting WRAP groups, and promote opportunities to enhance the ability of consumers served to move into recovery
• Arrange crisis intervention as needed
