About The Position
The Patient Navigator will be part of the Citywide Peer Team at UCSF/SFGH, providing intensive case management services to adults with severe mental illness and co-occurring substance abuse disorders. This role involves supporting clients in community integration, advocating for their needs, and collaborating with case managers to enhance treatment engagement. The Patient Navigator will utilize personal experience in mental health recovery to serve as a role model and assist clients in accessing various resources.
