The peer-client relationship and services will continue post-discharge. Peer Bridger work as an integral part of the FCS Supportive Services and Pioneer teams to provide trauma-informed, strengths-based services and support. The Peer Bridger functions as a role model through sharing their own lived experience with behavioral health challenges.

Requirements

  • High school diploma/GED
  • Must obtain and maintain an Agency Affiliated Counselor credential (Must apply within 7 days of employment).
  • Must be approved by Washington state HCA to bill for services
  • Lived experience of substance use recovery and the willingness to share their own experiences
  • Ability to work flexible hours
  • Valid Washington driver’s license or the ability to travel via public transportation
  • Washington Certified Peer Specialist with at least two years’ experience working as a peer

Nice To Haves

  • Experience with state substance use residential treatment facility services
  • Experience working with people from diverse cultures

Responsibilities

  • Provides education, assistance and guidance in identifying and linking to community-based resources (primary care, mental health and or substance abuse treatment, housing, financial benefits, transportation, self-help groups, and relapse, etc.) for current and potential program participants
  • Regularly meets with program participants at the facility for initial engagement, relationship building, goal setting, and to provide support through and post-discharge
  • Builds supportive relationships with clients, emphasizing the development of trust, mutual respect, encouragement and emotional support through sharing lived experience as a person in recovery from behavioral health conditions
  • Promotes a sense of self-direction and self-advocacy
  • Helps program participants develop skills to facilitate and support trust-based relationships and engage/re-engage with family friends, and other social support networks
  • Helps the client identify and/or re-engage with family, friends, and other social support networks
  • Helps program participants develop and maintaining wellness plans for participants’ medical issues in accordance with their treatment recommendations
  • Works collaboratively with clinical teams to provide the client with a peer perspective and understanding of the discharge process and the clinical team’s recommendations
  • Leads and facilitates group support to clients (Recovery Group, WRAP, etc.)
  • Helps facilitate a "warm hand-off" to a community behavioral health provider, including sitting in on intake appointments (if requested by the client), helping the client to connect to peer support and other ancillary support services offered by the agency such as housing, employment, etc.
  • Helps program participants plan how they will successfully manage their life in the community
  • Recognizes, respects, and maintains professional boundaries with clients
  • Manages a caseload of 6 to 12 participants
  • Completes all documentation requirements, including data logs, clinical notes, etc., in a timely and accurate manner
  • Completes billing documentation in accordance with RCW standards
  • Must be available to perform the essential functions of this job whenever scheduled to work
  • Attends shift/staff meetings and actively participates in planning, coordination and communication of quality professional services
  • Utilizes critical thinking and judgment to identify the most appropriate client support
  • Ability to write clear and concise reports and correspondence
  • Promotes and models teamwork and collaboration with coworkers and clients
  • Maintains quality of care consistent with or exceeding standards set by federal, state, local, internal, and contract requirements
  • Assists in the development of treatment methodologies and educational offerings in accordance with client needs as determined by program leadership
  • Adheres to professional and ethical standards for behavioral health services
  • Collects client and program data as needed
  • Completes file audits to ensure accuracy and quality of documentation and services
