The Speech/Language Pathologist evaluates, develops, implements and documents an intervention plan. This position provides intervention, evaluation and documentation of progress toward goals. The SLP develops and documents a discharge plan, completes all documentation required by multiple service sites or facilities, and meets requirements of third party payers. Requirements

  • Speech, Language, Pathologist (SLP) | North Dakota State Board of Examiners on Audiology & Speech Language Pathology | Prior to Start Date
  • Certificate of Clinical Competence in Speech-Language Pathologist (CCC-SLP) | American Speech Language Associate (ASHA) | Within 24 Months of Start Date
  • Driver's License with Acceptable Driving Record | Driver's License - Current State Licensed | Prior to Start Date
  • Basic Life Support (BLS) | American Heart Association (AHA) | Within 90 Days of Start Date
  • Masters - Speech Pathology
  • Proficiency in reading, writing, and speaking English to ensure effective communication in the workplace and with patients, families, and team members.

Responsibilities

  • Evaluates, develops, and implements an intervention plan using methods that are effective and are meaningful to the patient’s plan of care through remediation/restoration, compensation/adaptation, disability prevention and health promotion.
  • Provides intervention, evaluation and documentation of goal progress, modifies intervention methods as need, and works with therapy team members to achieve patients’ goal.
  • Balances schedules at multiple sites daily and adjust schedules quickly to accommodate changes or new patient evaluations as necessary.
  • Meets requirements of third party payers by including appropriate documentation following window periods and other insurance guidelines.
  • Develops and documents a discharge plan when patient has reached goals, achieved maximum benefit from therapy or does not desire to continue service.
  • Provides appropriate follow-up resources, services and training of patient and family members as appropriate.
  • Utilizes a team approach in the delivery and management of services and patient care.
  • Develops and maintain relationships with key facility staff at all sites including, but not limited to, the administrator, director of nursing, MDS coordinator, and dietary manager as well as the restorative nursing staff.
  • Completes all documentation on time and within practice guidelines.
  • Performs pain assessment and takes appropriate action according to standard operating procedure.
  • Performs other duties as assigned or needed to meet the needs of the department/organization.
