About The Position

Position Overview: Sweet Pediatric Opportunity: Ypsilanti This case involves a resilient 1-year-old girl with a history of Trisomy 21 and several cardiac repairs (AVSD/PDA). She is currently preparing for discharge from the hospital, and we are building her "dream team" to ensure she has a safe and happy return home. The Clinical Details Patient: 1-year-old female. Care Needs: Trach/Vent management and monitoring for feeding intolerance. Medical Profile: History of respiratory failure and cardiac repairs. The Home Environment: You’ll be working in a very welcoming home with parents who are highly involved and helpful with her care. Pet Lovers Welcome: The household is home to 5 cats and 2 lizards (caged), so the ideal nurse must be very comfortable with pets! Available Shifts Days: 7:00 AM – 7:00 PM. Requirements

  • Active RN or LPN license in the state of Michigan
  • Experience in home health, private duty, or long-term care preferred
  • Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)
  • Strong communication skills and a passion for patient-centered care

Responsibilities

  • Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)
  • Monitor and document vitals, symptoms, and changes in condition
  • Assist with ADLs such as repositioning, hygiene, mobility, etc.
  • Support patient well-being with compassionate and attentive care
  • Collaborate with families, caregivers, and the healthcare team
