About The Position

This is a PRN role. Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members.

Requirements

• Bachelors Degree (Required)
• Physical Therapist License (Required)

Responsibilities

• Perform modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
• Provide effective instruction to the patient and family through standardized educational programs and/or individualized training.
• Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes.
• Select and perform the appropriate assessment procedures.
• Establish and update formal plan of care to address individual patient needs.
