About The Position

The Pediatric Palliative Care RN at Baystate Health is responsible for providing skilled nursing care to children with life-limiting illnesses and their families. This role involves collaboration with healthcare teams, maintaining pediatric competencies, and advocating for patients' educational rights. The RN will also manage a caseload, develop individualized care plans, and participate in interdisciplinary meetings to ensure comprehensive care delivery.

Responsibilities

  • Demonstrates knowledge of applicable laws and regulations, developmental stages, pharmacology, health promotion and disease prevention and treatment, best practice interventions, Baystate Home Health policies and procedures and end of life care through daily interactions with staff, physicians and patient/families.
  • Demonstrates knowledge of developmental stages for the applicable age groups (neonate, infant, toddler, adolescent) and applies developmental theories and concepts when implementing care.
  • Works closely with the clinical educator to maintain pediatric competencies according to department standards.
  • Participates in twice monthly interdisciplinary team IDG meetings to promote coordination of services.
  • Establishes realistic goals and develops plan of treatment in cooperation with the patient, family and members of the health care team.
  • Demonstrates excellent physical assessment and care planning skills, current knowledge of pharmacology and medication administration, leadership skills, decision making and ability in the coordination of patient care.
  • Acquires and maintains a working knowledge of all third party payers and regulatory bodies and agency policies on issues related to documentation and care provided.
  • Manages patient caseload, adjusting visit frequency as needed, setting priorities, and providing support to the family.
  • Develops an individualized plan of care based on patient, utilizing assessment data and standards of care while maintaining fiscal responsibility.
  • Maintains required patient documentation.
  • Coordinates patient care with relevant team members, including collaboration with community based care providers and other health service entities.
  • Participates in relevant system and community based work groups (ie: monthly Medical Home meeting; DPH Care Coordination meeting; PICU discharge planning meeting).
  • Participates in MA Pediatric Palliative Care Network (PPCN) trainings and meetings.
  • Participates in ongoing legislative advocacy on behalf of PPCN.
  • Participates in periodic special events for patients and families.
  • Acquires knowledge of basic educational rights of pediatric patients and actively advocates for appropriate school services.
