Chemik Healthcare Services Inc is seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to provide one‑on-one private duty nursing care for a pediatric patient in the home. This role allows you to focus on individualized care in a calm, family‑centered environment. Job duties include but are not limited to: Maintain compliance with agency policy and procedures. Follow Medicare and Medicaid regulations ( as applicable) .

Requirements

  • Be a Registered Nurse (RN) or Licensed Vocational Nurse(L.V.N.) with an active current license in Texas.
  • Minimum of atleast one year of nursing experience as a L.V.N or RN.
  • Work positively and favorably with patients, families, and staff.
  • MUST HAVE EXPERIENCE IN HOME HEALTH CARE

Nice To Haves

  • EXPERIENCE WITH CRADLEMRX SOFTWARE IS PREFERRED

Responsibilities

  • Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
  • Conduct on-going patient care and assessments.
  • Administration of prescribed medication, treatments, and therapies.
  • Coordination of care
  • Educate family members on patient clinical care to enhance positive outcomes
  • Preventative initiatives to protect quality of care for patient
  • Observe, assess, and document symptoms.
  • Monitor reactions and patient progress.
  • Coordinate patient services.
  • Notify the physician of any change in the patient’s condition.
  • Perform skill outline in the agency’s approved policy and procedure manual.
  • Update clinical records according to policy and procedures.
  • Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.
  • Adhere to Medicaid and Medicare regulations.
