About The Position

Requirements

  • Graduate from an Accredited School of Nursing or a Bachelor's degree with 4-6 years of related experience.
  • RN - Registered Nurse - State Licensure or Compact State Licensure required.
  • NP - Nurse Practitioner - Current State's Nurse Licensure required.
  • Resource Utilization Group (RUG) certification must be obtained within 90 days of hire.

Nice To Haves

  • Clinical RN nursing experience with direct patient care.
  • Experience in Pediatrics, NICU, PICU, Pediatric ER, or Home Health.
  • Experience in Case Management and Service Coordination for medical services.

Responsibilities

  • Perform care management duties to assess and coordinate medical and supporting services for pediatric members.
  • Develop personalized care plans for long-term care members and educate families on available services.
  • Evaluate service needs of high-risk members and recommend plans for optimal outcomes.
  • Continuously assess and update long-term care plans in collaboration with the care management team.
  • Monitor care plans and member status, revising as necessary to meet goals.
  • Review member data to identify trends and improve care quality in compliance with regulations.
  • Collaborate with healthcare providers to facilitate member services and treatments.
  • Document and maintain member information to ensure compliance with guidelines.
  • Educate members and families on disease processes and healthcare options.
  • Act as a liaison and advocate for members between families, physicians, and agencies.
  • Coordinate community resources and service authorizations for members.
  • Perform home visits to assess member needs and collaborate with resources.
  • Partner with leadership to enhance quality of care and service delivery.
  • Precept clinical new hires and provide guidance during their onboarding process.
