About The Position

Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join CapitalCare Pediatrics Troy, a long-standing pediatric practice dedicated to providing comprehensive, family-centered care for infants, children, and adolescents. POSITION OVERVIEW As an Advanced Practitioner at CapitalCare Pediatrics Troy, you will provide pediatric primary care in a collaborative, team-based environment alongside experienced pediatricians and Advanced Practice Providers. The practice emphasizes preventive care, developmental and behavioral health, and strong partnerships with families to support children’s overall well-being and long-term health.

Requirements

  • License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
  • Physician Assistant: Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
  • Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
  • Current certification by National Commission on Certification of Physician Assistants.
  • At least one year of experience as a physician assistant in ambulatory health care.
  • Current CPR certification.
  • Nurse Practitioner: Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
  • Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
  • Successful completion of pharmacology coursework acceptable to NYSED.
  • At least one year of experience as a nurse practitioner in ambulatory health care.
  • Current CPR certification.

Nice To Haves

  • Three years of professional nursing experience in an ambulatory setting preferred.

Responsibilities

  • Conduct comprehensive evaluations for pediatric patients, including well-child visits and acute care
  • Diagnose and manage acute and chronic pediatric conditions
  • Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals
  • Provide preventive care, immunizations, developmental screenings, and patient/family education
  • Collaborate closely with physicians, behavioral health resources, and support staff to ensure continuity of care
  • Maintain accurate, timely clinical documentation in accordance with organizational standards
