POSITION SUMMARY: The LPN Care Manager is an essential member of the Johns Hopkins All Children’s Care Network (JHACCN). They function as a resource for care coordination and care management partnering with other members of the team to address resource and health care needs for patients and families. They promote quality care using a collaborative process that coordinates, monitors, educates and evaluates services needed. Their work includes analyzing and sharing performance reports with practices, assisting with improvement actions, working with patients/families, providers and payers to close care gaps, providing documentation in the population health database, and other work to meet the goals of our population health initiatives. Work Location: St. Petersburg, FL (modified hybrid schedule after successfully completing orientation) Work Hours: 8a-4:30p

Requirements

  • Current and active Licensed Practical Nurse (LPN) in the State of Florida
  • Graduation from high school or the equivalent required.
  • Minimum of 3 years’ experience in a health care environment.

Nice To Haves

  • Associate Degree preferred.
  • Managed care experience preferred.
