About The Position

Sunshine Homecare a Licensed Home Health Agency with headquarters in New City ,NY is seeking an experienced and compassionate Pediatric Field Registered Nurse Supervisor to manage the care of medically fragile children with complex health care needs residing at home or while in school. This role is responsible for delivering high-quality nursing services in alignment with the agency's philosophy, policies, and Standards of Community Health Nursing Practice. The position involves training and supervising field staff, developing and implementing care plans, coordinating care, conducting home visits, ensuring optimal patient outcomes and compliance with all agency and regulatory requirements.

Requirements

  • Graduate of an accredited Registered Nursing (RN) program.
  • Active NY state licensure and registration as a Registered Professional Nurse.
  • A minimum of one year of home care experience is required.
  • A minimum of one year of Pediatric experience in acute or residential setting required

Nice To Haves

  • Knowledge of EMR systems such as HHAexchange, RN PAD helpful

Responsibilities

  • Develop, implement, and oversee patient care plans in accordance with physician orders.
  • Conduct home visits to initiate services, conduct patient assessments and supervise field staff in accordance with regulatory requirements and agency policies.
  • Collaborate with Human Resources for onboarding and training of new staff, and ongoing performance management of field staff.
  • Coordinate ongoing home care services, ensuring effective and timely communication with field staff, caregivers, schools, physicians, insurance and other care providers.
  • Maintain thorough, accurate, timely and compliant documentation.
  • Monitor field staff documentation for timeliness, accuracy and progress towards goals.
  • Supervise and train field staff to ensure compliance with care plans, time and attendance and annual compliance with agency requirements.
  • Collaborate with intake and scheduling teams to ensure services are initiated timely and provided in accordance with the plan of care
  • Adhere to company policies, procedures and regulatory requirements.
  • Demonstrate professionalism in attitude, communication, and attire
  • Other duties as assigned by the Director of Pediatric Services
