As part of the pediatric team, the Pediatric Nurse Case Manager will provide skilled nursing assessment, planning, and care to maximize the comfort and health of pediatric patients and families in compliance with organization policies and procedures and applicable laws and regulations. Schedule: 4 days a week, Thursdays are required, other 3 days during the week are flexible; 8:30am - 5:00pm Location: Marin and Alameda Counties By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • One year experience as a professional nurse in the past three years or Bachelor’s degree in Nursing from an accredited School of Nursing
  • One year of pediatric nursing experience
  • Current license as a registered professional nurse in the State of California
  • ELNEC Pediatric training preferred and required within the first year of employment

Nice To Haves

  • Experience in acute care/ home health and/or hospice setting preferred
  • Public Health Nursing certification highly desirable
  • HPNA certification preferred

Responsibilities

  • Assume primary responsibility for pediatric patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
  • As applicable, apply specific criteria for admission and re-certification to hospice care or hospice waiver care to establish appropriate levels of care and patient’s eligibility for children
  • Initiate communication with attending physicians, other hospice staff members, facility staff members as applicable, and other agencies as needed to coordinate optimal care and use of resources for the patient/family
  • Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions
