Peak Health is seeking Traveling Health & Wellness Nurse Coach in Atlanta, GA. Coverage areas: Downtown Atlanta and Northeast Atlanta Work Schedule: 24 hours/3 days per week.

Requirements

• Valid Registered Nursing License for state(s) in which they meet program participants.
• Valid CPR certification.
• Valid Driver's License.
• Working vehicle and current personal auto insurance.
• Demonstrated proficiency in basic computer applications.
• Excellent motivational and coaching skills.

Responsibilities

• Travel to assigned client employment site to provide health evaluations for the participants.
• Review the individually completed Health History Questionnaire.
• Review and interpret individual blood lab test results.
• Obtain individual weight, blood pressure, pulse, body fat or body mass, and fitness level.
• Establish individual goals by making recommendations for improving the individual's health risk.
• Conduct evaluation via teleconference when no site is available.
• Perform other tasks as assigned by Nurse Manager.
