About The Position

The PD Registered Pharmacist is responsible for the provision of pharmaceutical care to all patient age groups (including geriatric and adult). The position is responsible for the preparation, dispensing, and monitoring of both the prescribing and effectiveness of all medications and IV admixtures. The PD Registered Staff Pharmacist performs all duties concerning the practice of pharmacy, in accordance with State and federal regulations and the established policies and procedures of the department. All methods of practice shall conform to written hospital policies and procedures and be carried out to the highest achievable level of competency in a manner demonstrating concern and consideration. All Registered Staff Pharmacists will demonstrate the Behavioral Health Center's philosophy of Total Quality Management by actively applying that philosophy in all dealings with both internal and external customers.

Requirements

  • Minimum Bachelor of Pharmacy from an accredited school of pharmacy
  • At least 1 year of hospital pharmacy experience
  • Currently Registered as a Pharmacist with the NJ Board of Pharmacy

Responsibilities

  • Assumes the role of drug information provider to all members of the health care team specific to patient populations and age groups
  • Evaluates and reports Adverse Drug Reactions, medications errors, interventions, and falls data
  • Participates in on-going DUES, medication monitoring, and Quality projects
  • Monitors specific medication prescribing in accordance with established drug usage criteria, prior to dispensing, as defined by the Pharmacy & Therapeutics Committee
  • Notifies prescriber of any changes/modification of medications prescribed
  • Oversees preparation of PYXIS fills, loads and refills, medication ordering from pharmacy wholesaler
  • Supervises pharmacy resident, pharmacy students, technical staff and volunteers to ensure accuracy and compliance with departmental policies and procedures
  • Utilizes the medication screening process to insure medication orders are complete, appropriate, within legal parameters and prioritized
  • Reviews orders for drug-drug interactions, drug duplicates, allergies, dosing appropriateness and scheduling
  • Willingness and flexibility to work alternating and rotating work schedules on short notice
  • Willingness to work with the realization that errors may have serious consequences to patients
  • Performs all other duties as assigned
