This is on-call as needed position Pay rates for Union positions are set in the Collective Bargaining Agreement with SEIU Local 521. Each position has a total of six steps, Step 1 (entry level) through Step 6 (15 years of employment with the organization). Union staff are promoted from Step 1 to Step 2 upon their one-year anniversary. The current Step 1 rate for this position is $35.40/hr, and the current Step 6 rate for this position is $39.09/hr. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Coordinate and provide direct services within a work team, ensuring that service delivery, including evaluation, crisis intervention, vocational rehabilitation, benefits coordination, closure planning, and communication of significant information, meets the needs of clients seeking employment; coordinate these activities with the DOR. 1. Assess client needs and resources and develop and/or implement individualized employment service plans, identifying strengths, barriers, preferences and abilities. 2. Facilitate structured Situational Assessment activities and develop summarized reports for DOR planning purposes. 3. Provide assistance and guidance with job preparation and other related support services to assist clients in reducing barriers to obtain and successfully maintain employment and to prepare for active job search. 1. Employment preparation activities include resume writing, filling out job applications, performing practice interviews, and arranging informational interviews. 2. Counseling and guidance on issues that may interfere with attaining employment success. 4. In collaboration with team members, utilizing a targeted, field-based approach, initiate, develop, and maintain relationships with community employers that lead to employment opportunities that match program participants’ strengths, abilities, and preferences. 5. Develop and link participants to opportunities for interacting with potential employers, members of the business community and other resource persons to support employment success. 6. Coordinate and provide services to support clients in maintaining employment through job coaching at the client’s worksite or offsite retention support, per client’s preference. 7. Support clients with benefit issues related to employment. 8. Provide linkage to outside resources. 2. Maintain communication with DOR counselor, mental health provider and other client-identified supports to ensure coordinated delivery of services. 3. Complete required documentation in accordance with agency, payor and regulatory requirements, utilizing computer technology. 4. Provide on-call coverage, as assigned. 5. Provide transportation for clients in personal or agency vehicle on agency business. OTHER DUTIES AND RESPONSIBILITIES: 1. Attend and participate in staff, program, agency and community meetings, as required, requested or assigned.
