About The Position

The role involves following the nursing process, which includes assessment, planning, implementation, and evaluation. This process is ongoing and involves collecting and analyzing healthcare consumer data (psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors), setting measurable goals in collaboration with the healthcare consumer and multidisciplinary team, implementing and/or modifying the plan of care using assessment data and evidence-based practice (including care coordination, education, and discharge planning), and evaluating progress towards identified goals with necessary revisions. The nurse is accountable for care directly, indirectly, or through delegation, adhering to professional and legal standards, and documenting all phases of the nursing process.

Requirements

Graduate of an RN nursing program
Registered Nurse: NE license upon hire
Basic Life Support - CPR certification upon hire
Advanced Cardiac Life Support certification within 5 months of hire

Set measurable and achievable short and long term goals according to standards of care based on the assessment by the nurse in collaboration with the healthcare consumer and multidisciplinary team.
Implement and/or modify the plan of care using assessment data, clinical reasoning based upon standards of care and evidence based practice, including care coordination, education and discharge planning.
Be accountable for the care directly, indirectly or through delegation, following professional and legal standards for delegation.
Use a systematic ongoing process to evaluate progress towards identified healthcare consumer goals.
Revise the plan of care when needed, involving the healthcare consumer and the multidisciplinary team.
Document all phases of the nursing process in the healthcare consumer’s record.
