About The Position

You represent the Legacy mission in all you do. Your strong interpersonal and relationship-building skills inspire your work providing transportation for our patients. In short, you help make life better for others. The Patient Transportation Coordinator is responsible for reviewing and coordinating non-emergent medical transportation requests for patients leaving the hospital and reviewing and coordinating acute patient transport within the hospital system. You will work closely with various stakeholders (internal and external) to coordinate transportation logistics and prioritize patient needs. Operating within our 24/7 lean department, you will be an integral part of our continuous improvement efforts, identifying opportunities to streamline transportation processes and enhance operational efficiency. Your proactive approach to problem-solving and your commitment to excellence will contribute to our overarching goal of delivering exceptional patient care.

Requirements

  • High School diploma or equivalent required.
  • Two years of healthcare work environment experience required.
  • Strong organizational skills
  • Knowledge of community resources and the social service delivery system
  • Excellent organization, oral, and written communication skills for effective interaction with various stakeholders
  • Ability to foster communication and build relationships among the treatment team
  • Excellent time management skills
  • Computer literate and ability to utilize Microsoft programs and Electronic Medical Records System (Epic)
  • Data management
  • Initiate coordination
  • Investigate and create new transportation pathways.

Nice To Haves

  • Associate's degree in healthcare administration, business, behavioral health, social services or a related field preferred.
