The Patient Services Coordinator will initiate initial contacts with patients and families, obtain verbal consent for NP visits, assist with back-office tasks in Homecare Homebase (HCHB), ensure timely scheduling of patient visits, prepare weekly clinician schedules, process new referrals, manage scheduling issues, monitor hospitalized patients, complete requested schedules, support branch operations, maintain medical records, run reports, and assist with patient transfers.
