About The Position

The Patient Services Coordinator-LPN is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management. This position manages schedules for all patients, edits schedules for agents calling in sick, and ensures patients are reassigned timely. The coordinator initiates infection control forms as needed and serves as backup during busy times, including receiving calls from field staff and assisting with weekly case conferences. The role includes maintaining the client hospitalization log, completing requested schedules, and ensuring supervisory visits are scheduled. The coordinator also prepares weekly agent schedules and assists with internal transfers of patients between branch offices. If clinical, the coordinator may receive lab reports, assess for normality, and perform patient visits as necessary.

Requirements

  • Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
  • At least 1 year of home health experience.
  • Prior packet review/QI experience preferred.
  • Coding certification is preferred.
  • Valid state driver's license and automobile liability insurance.
  • Excellent communication skills and ability to interact with a diverse group.
  • Strong organizational skills and ability to manage multiple assignments.

Responsibilities

  • Manage schedules for all patients and edit schedules for agents calling in sick.
  • Initiate infection control forms and send completed reports to HRD.
  • Serve as backup during lunch hours and busy times, assisting with calls and case conferences.
  • Maintain the client hospitalization log and enter coordination notes.
  • Complete requested schedules for skilled nurse/injection visits and other add-ons.
  • Process reassigned and rescheduled visits.
  • Run scheduling reports including Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepare weekly agent schedules and perform initial reviews for productivity/geographic issues.
  • Assist with internal transfer of patients between branch offices.
  • Receive lab reports and assess for normality if clinical.
