About The Position

The DDI Patient Navigator provides a single point of contact to patients and the multidisciplinary team through which care is coordinated to achieve optimal patient outcomes. This navigator helps bridge the gaps between hospital- based services, interventional procedures and outpatient-based services to ensure the provision of appropriate patient care. Provides tailored education and support to patients and caregivers regarding diagnosis, treatment options, along with collaborating with physicians/APPs to help assist with patient management. Provides relevant follow-up after discharge from all interventional procedures and hospital stays to ensure patient safety and satisfaction; adjusts care delivery model based on multidisciplinary staff input and scheduling clinic visit appointments. Acts as a point of contact on day of intervention, assisting patients through the inpatient stay, discharge, and post discharge appointments. Review, manage and process all referrals, best practice alerts and potential external new patients to ensure appropriate follow-up. Assist with service line quality projects, patient educational materials, patient outcomes and serves as a liaison for outside providers and community groups. This role will directly support our DDI Service Line.

Requirements

  • Depending on the needs of the service-line area, must have (1) a Bachelors in Nursing (BSN) from an accredited School of Nursing (or must obtain BSN within 2 years of hire) with licensure to practice as a Registered Nurse by the State of Florida, or (2) a MSW degree from a CSWE accredited college or Social Work program or (3) a Master’s degree in Health Administration or related field.
  • Three years related work experience required.
  • BLS Required.

Nice To Haves

  • ACLS Preferred.
  • Digestive Diseases, Gastroenterology, or GI Surgery experience preferred.
