About The Position

Hartford HealthCare, a comprehensive healthcare network in Connecticut, is seeking a Patient Service Coordinator for its HomeCare division, the largest provider of homecare services in the state for over 115 years. The Patient Service Representative is responsible for delivering professional customer service to ensure operational efficiency and a positive patient experience. This includes managing patient scheduling, pre-registration, registration, and insurance verification, as well as ensuring patients are cleared for HomeCare visits. The role also involves a variety of secretarial, billing, and clerical duties, such as utilizing the electronic medical record (EMR) for appointments and charges, coordinating referrals, preparing patient paperwork, collecting information, and answering phones. Additionally, the position provides administrative support to office staff and covers front desk/reception duties.

Requirements

  • High School Diploma or Equivalent required
  • Experience in a Customer Service role
  • Comfort and ability to handle confidential information
  • Comfortable working in a fast paced, evolving customer service environment with patience, flexibility and attention to detail
  • Proficient skills in windows-based software (including word processing, spreadsheet, database and presentation programs)
  • Excellent interpersonal skills necessary to communicate with departments, employees, physicians and managers
  • Problem solving skills
  • Engaging, self-motivated, and entrepreneurial

Responsibilities

  • Provide professional customer service to ensure operational efficiency and promote a positive patient experience

  • Ensure patients have been cleared for HomeCare visits
  • Perform a variety of secretarial, billing and clerical duties
  • Utilize the electronic medical record (EMR) to schedule follow up and specialty appointments and process patient charges
  • Coordinate referrals
  • Prepare the proper paperwork and collect pertinent patient information
  • Answer phones
  • Provide administrative support to the office staff
  • Greet all patients and visitors
  • Answer incoming phone calls in a professional manner, and triage the call if appropriate to ensure the patients’ needs are met in a timely manner
  • Create and distribute electronic messages in the EMR
  • Mail information to new patients prior to visit in accordance with department policies
  • Assist with general office duties such as photocopying, scanning information into a patient’s EMR, distributing reports, sorting mail, faxes, delivering materials, etc.
  • Provide back-up support to other front desk staff as needed
  • Responsible for mail processes, postage equipment, and office equipment
  • Maintain inventory processes including ordering and receiving according to appropriate levels set by Agency guidelines
  • Coordinate and clarify data with care management team in regards to medical supplies
  • Identify, research, and resolve order discrepancies
  • Effectively organize and maintain documentation for inventory control purposes and budgetary need pertaining to medical & office supplies/forms
  • Cover for other departments as necessary
  • Actively participate in the performance improvement process known as H3W
  • Perform other duties as assigned
