FTE: 1.0, Shift: Day, Schedule: Monday - Friday, 7:30am - 4:00pm In this role, you will play a vital part in supporting our Hospice nurses, social workers, and chaplains who are out in the field meeting the needs of patients and their loved ones. You will actively monitor team schedules and help coordinate adjustments throughout the day to ensure care is delivered promptly and compassionately. This is a highly supportive environment where teamwork is essential. You will help make sure every patient receives the care they need—whether that means assisting with schedule changes, helping to facilitate emergent admissions. Position Summary The Patient Services Coordinator is the initial point of contact for patients at our outpatient clinics. This role performs a variety of critical functions to maintain the clinical flow and to ensure patients receive care as ordered in a timely fashion.
