About The Position

Under the general supervision of the Manager of Patient Safety, plans, conducts, and supports activities involving patient safety. Compiles, analyzes and evaluates data using investigative, statistical methods, benchmark data, best practices or other comparative measures. Leads and facilitates teams related to risk and patient safety activities. Supports and assists the development, education and maintenance of the patient safety programs while striving for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.

Requirements

  • Bachelor's degree in a health related field.
  • RN required.
  • Three to five years in healthcare risk management, quality management, clinical documentation and/or accreditation readiness preferred.
  • Candidates with at least three years of recent clinical experience may be considered.
  • Must have experience working with computers including basic proficiency with Microsoft Office applications.

Responsibilities

  • Understands and utilizes the risk management process.
  • Participates in organizational and/or departmental risk assessments.
  • Provides assistance with goal setting, implementation of action plans, and the collection/analysis of data.
  • Investigates and analyzes actual and potential events and risks at MHC that could lead to loss.
  • Assesses liability and probability of legal action and notifies Manager of Risk Management of potentially compensable events.
  • Leads or participates with the completion of root cause analysis on sentinel events and failure mode effect and analysis.
  • Facilitates the development of related action plans.
  • Completes regulatory/licensure adverse event reporting and related action plans as required.
  • Monitors completion of identified action items.
  • Participates in the peer review process of the organization as needed.
  • Participates with the Director of Quality and Safety in the implementation and communication of the MHC Performance Improvement/ Patient Safety / Risk Management Plan.
  • Responsible for maintaining the organization's occurrence reporting system.
  • Analyzes data for trends and improvement opportunities.
  • Creates reports reflecting occurrence data for the organization.
  • Responsible for completing data entry into the MHA Patient Safety Organization system.
  • Monitors PSO findings for improvement and educational opportunities.
  • Assures timely and accurate completion of assigned reports.
  • Monitors organizational compliance with regulatory bodies (Federal and State), professional organizations, accrediting bodies, including the Joint Commission's National Patient Safety Goals.
  • Conducts chart reviews as requested, analyzes data for trends and improvement opportunities.
  • Collaborates with other departments to identify and address the CMS Hospital Acquired Conditions and the AHRQ Patient Safety Indicators.
  • Assists Manager of Patient Safety with the planning, preparation and presentation of patient safety education for MHC employees, the Medical Staff and others as appropriate.
  • Acts as a resource to MHC Departments and Medical Staff in patient safety issues.
  • Responds to or assists the Patient Advocate to respond to letters of complaint that relate to patient safety issues.
  • Researches literature to support risk reduction and patient safety activities.
  • Ensures that the use and retention of patient safety information meets established policies.
  • Works collaboratively with leadership, patient relations, patients, and patient's families to reduce risk exposure for individual incidents.
  • Actively participates in assigned committees/task forces.
  • Assists in the development of goals and objectives, policies, procedures and protocols as they relate to patient safety.
  • Demonstrates knowledge of and supports hospital mission, vision, value statements, standards of behavior, policies, operating instructions, confidentiality statements, corporate compliance plan and the code of ethical behavior.
  • Enhances organizational, departmental and personal growth through participation in external programs.
  • Performs other related duties as required or requested.
