Patient Retention Specialist A professional who assists in the prevention of acute care hospitalization for patients who are identified as high risk for re-hospitalization. Patient Retention Specialist will also assist the Director of Quality Assurance in obtaining records needed to complete ICD-10 coding. 1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing; 2. A qualified health care professional. 3. Three to five (3-5) years of health care experience, preferred. 4. One (1) year experience in home care, preferred. 5. Ability to exercise initiative and independent judgment. 6. Ability to work with individuals, to enlist cooperation of many people to perform/achieve a common goal. Works indoors in the home health office.
