About The Position

The Patient Referral Coordinator oversees and coordinates patient referrals to ensure timely, accurate, and seamless care and follow-up. This role works collaboratively with providers, patients, and external partners to facilitate the referral process, maintain precise documentation, and promote continuity of care across the patient journey. We’re seeking a detail-oriented, adaptable professional with strong communication skills who thrives in a fast-paced environment. The ideal candidate is comfortable with EMR systems, works well with others, and is committed to accuracy, confidentiality, and a patient-centered experience. As a special taxing district, we are accountable to our community, allowing us to prioritize exceptional patient-centered care over financial bottom lines. Our main hospital campus is located in picturesque Rifle, Colorado with twenty-five beds providing a wide range of services, along with a long-term care center and a satellite campus. While the secret is out that this is a great place to live, the atmosphere of a caring and friendly community has been preserved amidst a steady growth of population.

Requirements

  • High school diploma or equivalent preferred
  • 2-3 years of experience in a clinic setting preferred

Responsibilities

  • Coordinate and track patient referrals from initiation through completion, ensuring timely and seamless care
  • Verify patient demographic, insurance, and clinical information, and obtain prior authorization as needed
  • Communicate with patients to explain referral details, next steps, and assist with scheduling appointments and follow ups
  • Maintain accurate documentation in the electronic medical record, including referral status and related communications
  • Collaborate with providers, clinical teams, and external partners to support effective care coordination and continuity of care
  • Performs a variety of tasks and other duties as assigned
